John A. Hartford Foundation Awards $5.2 Million for Dementia, Senior Care
The John A. Hartford Foundation in New York City has announced grants totaling more than $5.2 million in support of evidence-based approaches to better care for older adults who have dementia, need hospital care, experience elder mistreatment, or have other complex health and social needs.
Recipients include the Icahn School of Medicine at Mount Sinai, which was awarded $2 million over two years to support an expansion of its Hospital at Home care model through the development of new implementation tools and resources, a national users group, and new payment strategies; the Education Development Center (EDC), which will receive $1.55 million over thirty months to implement and evaluate the Elder Mistreatment Emergency Department Care Model within six emergency departments; and the University of California, Los Angeles, which was awarded $1.51 million over three years to partner with EDC, the Gerontological Advanced Practice Nurses Association, the Alzheimer's Association, the American Geriatrics Society, and Hebrew SeniorLife to implement its Dementia Care Program and educate nurse practitioners in dementia care management at eight to ten locations...
