Elmezzi Foundation Supports Population Health Management Program at Mount Sinai Queens
With the Support of the Elmezzi Foundation, Mount Sinai Queens (MSQ) has continued to grow its Population Health Management Program by expanding health screening and care coordination services to several community based clinics in Western Queens and the MSQ Emergency Department.
MSQ strives to increase vital preventative and supportive services extending beyond the hospital and clinic walls to patients who are at risk for medical, behavioral or social health stressors by connecting them with health management and community resource navigation support.
Since inception in 2015, MSQ has successfully engaged over 2,000 patients and has provided longitudinal care coordination services to approximately 350 Queens’ residents, reducing avoidable emergency room and hospital admissions by just over 5%...
