Aetna Foundation Addresses Crucial Need for Localized Care
The key to truly transforming care is reaching people where they live and work, in a manner that works best for them.
Aetna recently launched a new program called Aetna Community Care, designed to provide local, in-person, personalized support by creating customized care plans that meet individual needs for participating members. In select regions, field care managers serve as “clinical quarterbacks” who meet face-to-face in members’ homes, local hospitals, nursing facilities or even local libraries (depending on where the member is most comfortable).
These clinical quarterbacks then work with members, their providers and community groups to connect them with community resources – such as Meals on Wheels, for example – and develop action plans based on member goals, helping them overcome barriers to achieve their best health....
