Position Summary:
Trinity Church is a vibrant, welcoming faith community in downtown Manhattan, with a congregation of 2,400 people. We have a mission to serve and care for our parishioners, neighbors, and the visitors to our Compassion Meals and Market. We support the community at large through over 500 events, programs and services. 

The Community Care Lead (CCL) has three primary areas of focus: 1) support for clergy, lay and outreach staff in their service provision, with case consultation, de-escalation, and resource generation; 2) directly engage and respond to complex cases that may enter Trinity through the congregation and Compassion Ministries; and 3) program management for resource and referral services. 

The salary for this role ranges from: $100,000 to $125,000. 

Essential Duties and Responsibilities:

Direct service provision and consultation (40%)

  • Provide consultation, training, and support to clergy and ministry leaders in their work with vulnerable individuals. Duties may include assistance with de-escalation and referral to care for time-sensitive situations.
  • Directly respond to individual Trinity parishioner and neighborhood care requests, for a limited number of on-going, high need cases, making effective referral to other caseworkers.
  • Visit parishioners who are in institutional facilities (hospital, rehabilitation center, nursing homes) to advocate for care.
  • Liase with Neighborhood Support and ministry staff for in-person presence at open office hours in Trinity Commons, Compassion Market and Meals, and at Sunday coffee hours. 

Program management (40%)

  • Assess and design standard processes across Parish Life and Neighborhood Support for intake, needs identification, and care team assembly/referral.
  • Design tracking and reporting processes, utilizing Trinity’s Touchpoint database (“My Trinity”), for scaleable practices and effective follow-up.
  • Establish clear boundaries of care; including, but not limited to referral, direct care, and development of policies and procedures for declining services when appropriate.
  • Collate and codify existing Trinity resources for vulnerable populations.
  • Leverage existing partnerships and cultivate new relationships with external organizations to expand Trinity’s capacity to respond to community needs. 

GENERAL RESPONSIBILITIES (20%)

  • Attend Neighborhood Support and Parish Life Ministry offsite events as needed.
  • Adhere to confidentiality rules and all other Trinity policies, procedures, and rules.
  • Perform all duties in a manner that promotes Trinity’s mission and Ways of Being.
  • Assume other related responsibilities and special projects as required.

Required Knowledge, Skills, and Activities:

  • Case management experience including engagement, intake, assessment, referral, and evaluation. 
  • Track record of advocacy for high need populations (i.e., policy and resource development)
  • An established network of resources and connections in the social services community
  • Program management experience including designing systems and data collection.
  • Community oriented and collaborative 
  • Outstanding listening and communication skills
  • Ability to take initiative and demonstrate perseverance and resourcefulness to deliver outcomes
  • Experience working in communities of color and senior populations
  • Experience helping people navigate budgetary crisis, eviction, and debt
  • Ability to manage and prioritize job responsibilities and navigate systems within a complex organization
  • Outstanding judgement and discretion in handling sensitive matters
  • Motivational interviewing experience

Required and Preferred Education, Experience, and Credentials:

  • Master’s degree in Social work, Sociology, Psychology, or other related fields or an equivalent combination of training and/or experience. 
  • Bi-lingual or conversational Spanish
  • Minimum of five years experience in single stop, governmental, or other non-profit community social service setting
  • Knowledge of religious community life and spiritual care
  • Working familiarity with internal family and church systems theory
